To get started make sure you have your son and daughter with you as they will need to sign some of the forms before you can submit them. You will also need you son/daughter student ID number.

1. To access the online forms hold your curser over the “Electronic Participation Forms” tab.

2. Click on the form name and fill out the information requested. Parent/Guardian will be asked to provide information such as home phone numbers, cell phone numbers, email addresses, name of primary physician, insurance policy information and other pertinent information. Please make sure you have this with you when filling out your forms electronically. If for example you don’t have insurance then don’t leave the space blank put “NONE”. If you leave a space blank or you or your child doesn’t sign the form you will not be allowed to submit the form.

3. To sign the document, click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature.” If you make a mistake and need to start over, click on the refresh icon next to the signature box. If you are using an IPAD, you may create an electronic signature, by holding your finger to the screen and signing your name or by using a stylus.

4. Once you have filled out all of the information on each page you will have the opportunity to print the document.

5. You will receive a confirmation email once the document(s) have been reviewed by Hardin - Jefferson ISD.

6. In order for your child to participate in athletics you must sign and submit every document under the “Electronic Participation Forms” tab along with a completed Physical/Medical History filled out by you and a Dr of your choosing. High School students have to fill out all forms. Students that will be playing Middle School athletics have to complete all forms except for the “UIL Steroid Form”.

7. After you submit your online forms you can print out the Medical History/Physical Exam form by holding your cursor over the Physical/Medical History tab. A drop down tab will appear. Click on the drop down tab and the Medical History/Physical Exam form will appear and you will be able to print it out.
